Email script on homepage only works for 1 of 2 pages

Hi Incomedia,
These are the settings for our homepage regarding sending/recieving email from the 2 pages concerning "Apply for membership" and for "Discontinue membership".
Only the Apply-page is sending the proper email to us together with the collected data. The Discontinue-page does not??
In my opinion it is the same setup for both pages.
What do we do wrong? Are also checking this with the provider now.

The online panel is regular ...?... www.yoursite.com/admin/
Did you do all the various MAIL TEST ...?...
The sender's e-mail is the one associated with the same web space ...?...
(IT) - Il pannello online è regolare...?... www.tuosito.com/admin/
Hai effettuato tutti i vari TEST MAIL...?...
L'e-mail del mittente è quella associata allo stesso spazio web...?...
