E-Mail form address input

When creating an E-Mail form, I'm not able to select anything for Sender E-Mail address under Data submission, and I also can't select anything for User E-Mail address under Confirmation E-Mail.
These items are both shown as drop-down menus and there is nothing in there to select!
Please help!

Just to clarify, I'm using Website X5 v9.0.6.1775

Within the module object sending e-mail you will need to insert a type of e-mail field; in this mode you can see the drop-down list of the field e-mail
