Problem with send on email object

I have a problem when I'm creating the email form object.
After I input the sender email address both on the data Submission section and then on the "Confirmation email for the user" I then hit OK to take changes, I get the error message "enter all mandatory data to continue" How do I fix this?

Have you filled your url in, in General Settings.....
