Evolution 13 - Email Form Not Working

Ever since I upgraded to Evolution 13 none of my email forms are working. I have tried changing the email settings in the Date Management section. I was receiving emails on 10/15 and updated on 10/18 and now the email forms do not work. Any suggestions? Is this an error with Evolution 13?

I have also noticed that if you enable the 'captcha' anti-spam filter the boxes are not sized correctly and the user cannot see the word to type in the box for verification.
