Setting up a contact page

I am trying to set up a contact page whereby clients can just type in their email address, name, subject and message for enquiries.
I am failing to achieve this with the contact form icon, it only shows 1 line when am previewing the site.

Then you have done something wrong!
If you want to add more fields you have to press "add" and a new line will be inserted. You also have to label it.
