This is similar to a question posted earlier with a little more detail. I need to make a decision soon as to whether to go ahead and buy Professional. Extremely important is being able to create a form page as seen here: http://www.envirosnowmelt.com/CreditCardOrders.htm
This is a form that is filled out online and then printed and faxed.It includes radio buttons and several fields. It's different from a shopping cart in that it is filled out but not submitted online. Also, where is the .pdf users manual? A reply very soon would be much appreciated so I can move on.
What's with the tags field? I put somethiing in it but never seen that in a forum before.

The manual can be found online on http://help.websitex5.com/professional/en/
You will be able to create a form where you need to work on the layout to make it look like the one you linked but this form will be sent and is not saved as pdf file since the function to use is the email form object.
To save it as pdf file or to print it once filled you will need to use a custom script where you send the file which then will be saved and printed so it can be faxed.
