William, have you put your email address in the appropriate field?

My send tab looks like this (eMail is the field I have named for the users email address)

Be aware, that some providers demand the sender address is a mail address from the domain.

I tried all of the different optons for scripts and even tried the authenticated SMTP.  The email to the person who fills out the form goes through on almost all of them, but the email to me does not. I am using the same sender email for both.  I ended up using an online form builder because I could not get it to work.
