Contact us bottom in the header followed by Showbox contact form

Dear all,
In the header I want to add a Contact us button and when people click on it a Showbox must be opened with the contact form.
Is this possible and how?

Hello.Yes, I think it is possible.
First create a page without a template and place the form there.
Then put the button in the header and set the settings:
