Email problem

I have made an email response form but when the people respond some parts of the answers appear above the form, out of place. Example attached.
I have downloaded the latest version of websitex5 but I still have the same problem.
Any ideas ?

As Frank said, the attachment is missing.
Maybe your server is not allowed to send HTML emails. Did you read this? http://answers.websitex5.com/post/24269
