The program has no limits and no further costs since you can use it as long as you want and you can create as many websites you want.

The only cost will be the hosting in case you choose a paid hosting with a custom domain. If you use a free hosting you will have no further costs.

yes, you need to buy your host and your domain.

some of the versions comes with special discount or one year free host, but thats only good for one year, for the secound year you have to pay!
