I am having a problem adding an "E-Mail Form".

In filling out the "Data Submission Section" beside the Sender E-mail address there is a box and it keeps telling that this is a Mandatory Field, yet when I hover over it , it turns blue and it is not possible to write in this box and I cant fill it in. Also in the Confirmation E-Mail to the user the same thing happens in the User-e-mail box.
Please tell me how can I fill in these two Mandatory Fields?

In the list tab set a name for the email adres. this name can then be used in the sender email option in the send tab.
The mandatory field is the one below (user email adress) this should always be filled with an email and should be possiple to acces always.
If the above did not help please make a screenshot.
