Email not sending through site

I am using X5 13 Professional. I have added the email form to my site but it isn't functioning.
I have it set to use PHP Mailer. I can log into the backend of the site to test the system. All three tests show pass and I am able to successfully send a test email through the backend, but it will not send from the web page itself.
What else can I look at to see what could be causing the problem?

Have you set a subject in the email form in the send tab?
During the test in the admin backend did you test the PHP Mailer script by entering a subject or wtihout?
