Order Form Options Not All Displayed

On the Order Form, not all product options are displayed. E.g., if Options are, Size: S, M, L and Color: Black, Blue, Red, ONLY the Color is shown on the Order form (Size is not). Does anyone know how to make sure ALL product options are reflected on the order form? Thank you!
