Hi,
I recently purchased the "image form" object for my Pro version.
I created three Image Forms on a new unpublished site to try them out. All three worked fine yesterday. The progress bar worked fine and when all questions were answered, the contact form (generated in the Data Submission tab) would appear.
Today, none of the three objects will work. When I click them, they're frozen, and the progress bar is not visible even though I selected it in the Style tab. 
So I re-created new Image Forms from the beginning. The new ones show the progress bar but when all the questions are answered, the contact form does not appear.
Could you test the Image Form object to see if you have any similar problem? 
I like the object. However, may I make a suggestion to the designer? The object is designed to accommodate 4 multiple-choice answers. However, is it possible to design it with an option for 2 or 3 or 4 answers? 
It would help in cases where there are only 2 options, such as when the answer is primarily [Yes/No] or [True/False]. 
In any case, could you test it and should I uninstall the object and install it again?
