E-mail form not working, MySQL database failed to conect

Hello, I'm facing a strange issue, which occured in the last couple of weeks I think. EVO 12 is what I'm using now.
I'm running a website, which has a petition-form. My ISP uses smtp, I have all settings, including custom pop3 and smtp ports.
I had no issues , but suddenly it stopped working.The default script that worked was php mailer.
I get the notification message, but the e-mail can't get through. On site manager, I get failed test on sql database. I have rebuild it, same problem. Also tried smtp and authenticated smtp, no luck.
What could be the culprit? Thank you.

Found the solution, I had to recreate the form. My provider made some updates and the new protocols weren't fully implemented. Thanks for the feedback.
