I purchased the Poll Object for my site and am having difficulty getting it up and running.

I created a database on GoDaddy my host (image attached)

I completed the advanced setting database info (image attached)

I completed the survey info (image attached)

Everything seems to save. However as you can see it will not display. What am I missing?

Help please.
