For insert predefined countries and cities you should have a script HTML-Javascript which you can insert in an object HTML & Widgets. How you want appear telephone country?
You can use a database MySql but you should created a database. In WebSite X5 Evolution 9 you can insert data from object form email in a database MySql.
In WebSite X5 Evolution 9 you can insert at the step 4 an reserved area (for each user) and after you can associated the users with page where you want insert area login. For make this at the step 2 you can set your page as protected.
