Insert in the open field, there are two fields " attribute" and "database field name", I wonder how these fields and their properties? I have a mysql based, existed some form of integration of form with this base? Can anyone help me? please....

The <name> attribute allows you to specify the value to associate in the attribute in the field when the HTML code is generated on the Form. The database field name is the name for the field choosing because the data collected can be properly recognized and entered into a database. The field name of the database must be the field that you've provided you in the database.
