Form not working when hitting send. My host is godaddy

I made a website and My form Send button doesnt work. My website is www.feelgreatmike.com My domain is through GoDaddy out of the USA. And My host is GODaddy also.

Was the website also uploaded to the web space using the export function of the WebSiteX5 program?
Or was the website first exported to the hard drive and then to the web space using a separate FTP program?
It looks like you are opening the preview in WebSiteX5.
Below is the screenshot of the message that appeared in the top right corner of the browser when I tested the form and clicked "Send".

First I tried using the FTP to download my site.. I couldnt figure out what to put for Data Managment. So then I got a program called File Zilla to download my files that was made from Websitex5 program. Files are downloaded to Cpanel in which Go Daddy uses. Your last question I cant access that part since my Domain is through Go Daddy.

Domain at GoDaddy and where is the web hosting?
Normally, the entire website is uploaded (exported) to the web hosting provider's storage using the WebSiteX5 program, without the need for a separate program such as FileZilla.
An external program like FileZilla can cause problems and should only be used if you know exactly what you are doing.
Domain and storage space can be in one web hosting package, which is usually the case.
If the domain is with another provider, a connection to the web space provider is established via DNS.
In both cases, WebSiteX5 can be used to "export the website to the Internet" without a separate FTP program like FileZilla.

My web hosting is through Go Daddy also.. The reason I went with FileZilla was I couldnt figure out the setting under Data Manament to up load my site through your FTP. I cant switch my Domain to Website x5 until the middle of May. That rule is Go Daddy. So am I out of luck getting this resolved?

Depending on the web hosting provider, FTP, FTPS or SFTP may be required under “Server Type”.
For the emails or contact form, the “Server type for sending emails” could either be “PHP Mailer (recommended)” or “SMTP authentication” is required for security reasons.

you should be able to find the information to be used for the Data Management tab either in your hosting provider's control panel. They should also have guides on how to find these parameters, please take a look on their website.
