Conditional form fields

How do I insert a field in a form based on a condition from a selection list? If selection list "other" is selected, I'd like to insert, or reveal an addition field with label, stating please provide details. I could not find conditional form fields, but I'm new so may have missed it.

Can ypu add this to the requested feature list, long with a generalized database forms builder for more complex data colkection from a website. Also, please add the request to support American English for website spell checking and correction.

... selecting one of the two items in the select field, yes-no, would make a special hidden input field appear for filling in...

Hi KolAsim, Thank you for your response. I liked your Image form, but in my case I think that this would be overkill for just requesting a text input to when the "other" selection was made, but I will keep this in mind for future sites.
Where can I find more details on your DIV_SINO method please?

As long as Incomedi doesn't offer the feature for WebSite X5, the additional input field can still be added with a note, see example below
Maybe KolAsim provides an EXTRA code to hide the additional input field if "Other" is not checked.

Your selection for xyz
( ) Option 1
( ) Option 2
( ) Other
Enter here if "Other" was selected.
