Captcha SERVER error

Hi Please can you help me on my client feedback section of my webpage, the captcha section has been enabled but i am getting a SERVER error in the box and as a result i am unable to get any client feedback.
I have contacted Streamline who provide my webhosting and they say there is a sofware issue! If you have a look at my site you will know what i am talking about! http://yourdiscopartynight.co.uk/booking-info.html
Please can you check and see if there is any issues?

You will see that when using evolution x5 form, there seems to be a server problem in the captcha section . I been having this problem with the form now for few months and nobody seems to know what is causing the problem!!!

Error message:
