Il faut mettre le sous-répertoire du serveuroù enregistrer les données (modo de salvar os dados = repertório) et non PHP comme on le voit sur ta copie écran.
C'est ta page du Livre d'Or qui doit être définie en extension PHP...

If yes has this folder the CHMOD set to 777 (writing permission)?
