To and From Address on Email Form Since Upgrade

Since upgrading all the lead forms generated now come from the companies email address. The details in the body on the lead form are fine but you can no longer reply directly to them. Any suggestions on how to overcome this?

Hi it doesnt give me that option - it only gives the x2 options below! Maybe there is another setting that I am missing?
