I cant put user email address because its empty... look attachment.
and.. if you put many text field, then email shows only one.. it has some bug i think.
but what this first problem? thanks
i want confirmation email and that other email to work.

Hello,
The user email address is empty because you should add as type of field a field "e-mail". this field will be present in the tab send of object form email.
