Site visitor should be able to select and send a Google Location in a form along with Name, Email, Number etc.  en

I want to collect a norman data from website visitor like Name, Email, Number etc. 

Along with it, visitor should be able to send his / her Google map location. 

Please tell me how shall I do it?

You could ask customer to give it to you by filling in an (email) form
