Problem with captcha, email form and locked page

I've problem with captcha not working and keep on telling me it is incorrect. I've a few websites in the same server created using Website X5 Evolution 10. After updating the software and creating a new website (www.onlinemall2u.biz), the captcha got problem. Even the email form also not working (i.e not sending email and receiving email) and the locked page also got problem (i.e cannot log in even after I inputthe correct username and password - username : dramin / password : k8954577).
I don't have these problems before this when creating website using WebsiteX5 Evolution 10. My server using PHP ver. 5.3.28. Hope somebody here could help me solving these problems.

I do some amendment at my previous website contact form (www.passioncoffee.tk), previously the contact form is working perfectly. After I do some modification and reupload again, the contact form and captcha is not working anymore. This problem happen after I updated my software. Please help.

I tested the form and it works the captcha. Try to clear your browser cache and then try again.
You should have recieved the test email I sent you.
About the login form can you please tell me which page is locked?
