Upgrading my website from Evalution 15 to Professional 15

I created a website with Evalution 14. I purchased V15 Professional and uploaded the site. All went well in the conversion, until I used the site. The Communication Form failed. When clicking the send button it wiped out all the data on the dropdown boxes. Self input Fields were un affected. Has anybody else had the same problem and did you solve it. What hair I HAD HAS NOW GONE due to the frustration of trying to look for a solution.

The developers at Incomedia are aware of the issue and are working on a solution. Perhaps for the time being you can radio-buttons on your form?
