I'm afraid it wouldn't be at all easy to add an extra field to the built-in Guestbook in this way.  The Guestbook doesn't have editable fields like many of the other elements in X5.  Instead the Guestbook Object is encapsulated in the code, and its properties are hidden from view.

You could look for an alternative PHP-driven guestbook on the web, and incorporate it into your project with probably a minimum of work.

Another idea would be to simply use the Email Form Object, in which case you could add as many fields as you required.  The downside would be that you would have to manually publish any comments to your designated guestbook page (this would be an ordinary .html page, as you wouldn't be using the Guestbook Object in any way).  However, depending on how busy you expect your site to be, this might be entirely manageable.
