Form not sending

Forms on my website that have worked perfectly well in the past are no longer sending.
For example, the form on this page:
http://www.iwhealthgroup.co.uk/publication-of-results.html
When the user completes and sends, a blank screen appears:
http://www.iwhealthgroup.co.uk/imemail/imEmailForm_804841mx.php

The screenshot of your settings seems to be missing, but the most likely explanation for the blank screen is an outdated PHP installation on your server.
Version 2020 of WebSite X5 now requires PHP version 7.1. (or higher) in order for certain features like the Contact Forms to work properly:
Version 2020.1 requires PHP 7.1
Your webhost will be able to advise you as to what version is currently installed on your server.

Paul is right, your PHP is PHP/5.6.40... And If you test your admin panel you are on error 500:
http://www.iwhealthgroup.co.uk/admin/
Need to update PHP...
