How to create a page for "testimonials"

I want to create a page at http://lanesautodetailing.com/ where a customer can leave a comment or "testimonial and rating" about our sevice.
I tried a blog but can't seem to figure out how to make it work so comments can be left. Do you have a tutorial on this?

well I am stupid but don't see how to let anyone that gets to the site fill in info... I looked at the help and did not find stuff there.. do you have a sample that I can use and change the contact info as needed?
