Make two pages in Map Creation (Contact & Thank you what I’ve done)
Go to their Page Properties and make the page without the template
Then link to them using the show box feature……

I work on project for restaurant, and I did exactly as you said but it is a problem, because the Showbox show horizontal scrollbar ...???
I made the pages without the template (640 x 480), Showbox also of those size, now I do not know how to solve it, to disappear horisontal scrollbar ??

The solution proposed by Frank is the correct one. Just enlarge the popup size if it's not large enough to show the form properly.
