Sending attachment with e-mail form

In my email form I have also field "attach file". When I select file and then push send button, I get email with only other fields data without attachment. What is going wrong?

Check please what email script type you are using in the preferences on step 0 and if t here are any limits with your hosting provider on sending emails with the php mail () function.
